This form is designed to collect information on parents followed by a section for information on each child in the family attending Religious School.

Please be very careful when you select the grade your child will be attending. You may switch from first choice to second choice within a line; however, you will not be able to eliminate the selection of that grade line. If you make a mistake, please note your child's grade and first and second choice for session in the section at the end for comments and questions.
