In exchange for your participation, we will send you 2 samples (a 12 day supply and $24 value) for the cost of shipping.

Shipping is $4.95 for the US and NL (as the samples are packaged in glass vials.) Unfortunately, UPS will not deliver to PO boxes.

After you have successfully filled in the survey and clicked "Finish" you will be taken to the page where you can add the samples to your cart and check out.
