The information you provide here will be kept strictly confidential and will not be shared outside of United Way.

Information from all agencies that respond will be aggregated, however, so that we can better inform policymakers and the public about the serious challenge facing the network of community-based services in the Commonwealth.

Please note: This survey is intended to collect information regarding agencies experiencing delays in receiving payments under state or local government contracts. If your agency does not have such contracts, or has not experienced payment delays, you do not need to complete this survey.
