Children's Health Survey: Real Solutions for Real Problems


Thank you for helping me write this book on the health habits of our children.  I greatly appreciate your taking a few minutes to complete this brief survey, which will provide valuable information for my book. The experience and opinions of seasoned parents, teachers, coaches and health practitioners are priceless. There are only a few questions for each section.

My name is Chantelle DeShazer and I'm a licensed acupuncturist, a health advocate for children, and a mom. I'm reaching out to you because our children's health is in crisis. How can they think well, behave well or be happy when they don't feel well? This book will help the busy parent, babysitter or teacher find simple solutions and clear action steps to improve their children's health. This is an important opportunity for the voices of those on the front line of our children's development to be heard and to share real solutions to real challenges. It is my objective to create a simple and practical resource guide, as well as a book directed at young children so they can learn the value of great health. 

Many of you are both parents and coaches, teachers or health professionals. Please fill out the sections that apply to you and the ages of your children.  If your children are grown, please indicate what you did when your children were at the three separate age ranges.
