Please provide a brief description of each image submitted, including size and medium. Name your image files with your last name and first name followed by the image number (LastFirst01, Last First02, etc).

Participants must have a current KAC membership at the Patron Level ($150) at the time of application. If you are unsure of your current membership status or level, please contact KAC Development Director Lauren Erlinger at: lerlinger@kirklandartscenter.org or 425.822.7161 x 107. Current KAC members at a lower membership level need only to pay the difference to bring them up to the Patron Level. Memberships are good for one year.
My membership at the Patron ($150) level is current.
I will mail a check to KAC for my membership fee.
I will call KAC and pay for my membership with a credit card.

By completing this form and checking this box, I agree to the following:
1. To be present at the open studio listed on my application the entire time of the Kirkland Artists Studio Tour, May 8-9, 2010.
2. Each piece of work submitted during KAST will be my own original creation.
3. To collect and remit sales tax on all sales.
4. Any images I submit may be used for promotional purposes.
5. I have read the Prospectus and Guidelines for Participation and understand all the listed details.
6. I agree to contribute time and energy to the success of this event.
7. I agree to distribute up to 50 postcards announcing the event. I understand that Kirkland Artist Studio Tour (KAST) and Kirkland Arts Center (KAC) does not carry insurance to cover my personal property, or against personal injury to myself or any Tour visitors. I have been advised to obtain my own insurance for this event and I release KAST and KAC from any damages to personal property or injury (to myself or anyone else) resulting from the participation in the 2010 Kirkland Artists Studio Tour.
