We are thinking about a website redesign and wanted to make sure we include the feedback of actual dental patients. 

We want to make our website easy to navigate and to contain the right amount and mix of information a person looking for a dentist would want.  Please feel free to be as opinionated(and brutal) as you want and also include other websites that you have been to and like regardless of whether or not they are dental related.  

The survey is 12 Questions and is  Anonymous.
