In working with donors (primarily individual and corporate), I've been asked by a business to present community philanthropic opportunities at a meeting in mid-June. While I know of many organizations, I'd prefer to present an accurate slate of organizations and their volunteer and giving needs/opportunities.

Taking this as a challenge, I've designed a simple information survey form to help me build profiles on community organizations to aid my presentation.

Based on your responses and/or the request of this donor (and/or other potential supporters), I may be in contact with you for additional information or to schedule a meeting between your organization and the potential supporter.
