Survey for Parents of Gifted Children: Your Experience with Your Child's Doctor
Consent to be a Parent Research Participant
The purpose of this study is to improve medical services for gifted and talented children. The sponsor is SENG--Supporting Emotional Needs of the Gifted, a 501 (c) (3) nonprofit organization. SENG is the only national group solely committed to understanding and advocating for the social and emotional wellbeing of the gifted.
You are the parent of a gifted child, and that makes you uniquely qualified to advise us about how physicians meet the needs of your gifted child. This survey will take you about 10 to 15 minutes, and the questions relate to your experience with doctors when seeking support for your gifted child.
· This survey is confidential, anonymous, and voluntary. · No personal information is requested.
· You take no risk in answering the questions.
· There are no penalties if you choose not to participate.
· You may refuse to answer any question.
· You may withdraw participation at any time.
But please, be aware that your responses are valuable. By participating in this survey, you will help to educate doctors and the public about gifted and talented children.
Please email questions or comments about this study to GiftedChildSurvey@gmail.com, or contact SENG at office@SENGifted.org. For questions about your rights as a participant, contact the NEIU Human Subjects Committee at 773-442-5844.
