This survey is intended for family members, friends or guardians of people who participate in Community Job Placement programs out of the Reading, Beverly or Burlington sites, in competitive employment in the community.

We are sending you this survey because you are an important part of the life of someone in our Employment Services Program. Our primary goal is to be responsive to the needs of our individuals and their family members. One of the best ways to determine whether we are meeting this primary goal is to get formal feedback from the individuals, their families, or other people in their lives.
