This survey is intended for family, friends or guardians of people being supported by EMARC's Adult Family Care Program.

We are sending you this survey because you are an important part of the life of someone in our Adult Foster/Family Care Program. Our primary goal is to be responsive to the needs of the participants and their caregivers. One of the best ways to determine whether we are meeting this primary goal is to get formal feedback from participants, families, or other people in their lives.
Please take a few moments to complete this survey. Your input will help us to provide quality services that meet the needs of the individuals of EMARC, while allowing the maximum amount of independence and choice possible.
