This survey is intended for family members, friends or guardians of people in EMARC's Supported Living program, which is different that the 24-hour Group Home program. People in Supported Living may receive Mentor Support Hours through DDS, and can range from 4 hours a week to 20 hours a week.

We are sending you this survey because you are an important part of the life of someone in our Supported Living Program. Our primary goal is to be responsive to the needs of our individuals and their family members. One of the best ways to determine whether we are meeting this primary goal is to get formal feedback from the individuals, their families, or other people in their lives.

Please take a few moments to complete this survey. Your input will help us to provide quality services that meet the needs of the individuals of EMARC, while allowing the maximum amount of independence and choice possible.
