In early January 2012, you received an email at this address reminding you that the 2011 Dental Amalgam Program Annual Report was due January 31, 2012. This report collects information about your amalgam separator maintenance and amalgam waste disposal.

We have not received your report and your office is out of compliance with your Wastewater Discharge Permit. To keep from being subject to possible fines, please complete your report immediately!
