This customer survey is being sent to selected customers currently using ICIA labs (Conventional and/or Genetics Lab).  The goal of this survey is to gather candid feedback enabling us to improve the quality of the services to our customers.

We value your time and your opinion.  Upon receipt of your completed survey, when written comments are included, we will apply a $25 credit to your account.
