Instructions on the survey: If you prescribe, dispense, and/or administer opioid products (e.g. fentaNYL, HYDROmorphone, morphine, oxyCODONE), please take a few minutes to complete this brief survey about selection, dosing, and patient monitoring when using opioid products. This survey was developed by the Pennsylvania Patient Safety Authority in collaboration with the Pennsylvania Medical Society. The results will help identify opportunities for improvement and aid in the development of targeted, high-leverage strategies to improve the safe use of opioids in your facility. The information obtained in this survey is confidential and identifying information will not be shared or distributed publicly or among other Pennsylvania facilities or agencies. The data collected here will only be used by the Authority for educational or quality improvement projects. If you exit the survey and re-enter it at a later time you can pick up where you left off.
