Men's Impact Event Registration
Projected Informed Consent Document 

DESCRIPTION: You are invited to participate in a research study on how to develop the skills and motivation that will cause men to seek to be fathers of their unborn children. The research will provide skill training in fatherhood during a full day Impact Event training day, followed by opportunities for fellowship with other men through activities of their interest in Affinity Groups of like-minded men. The questions you will be asked in research surveys will discuss perceptions of your skill level in certain fathering tasks and your motivation to seek to be a father and to attempt to fulfill the role of a father.

Research will provide Care Net with feedback to know whether the Impact Events serve to prepare you for desired fatherhood roles and how it can be adjusted and modified to better support those objectives. You will be asked to complete three online surveys over the course of the Impact Event program and two program events. Your survey data will be analyzed in certain demographic groups and analyzed for assessment of the Impact Event effectiveness. Your personal information and data will be protected by the procedures in use at Care Net for use and storage of client data.
TIME INVOLVEMENT: Your participation will take approximately 10 hours of your time.
RISKS AND BENEFITS: The risks associated with this study are possible negative feelings about past or current decisions you made that would be different had you received the training and mentorship motivated and provided in the research study. The benefits which may reasonably be expected to result from this study are improved fathering motivation and skills that will benefit you and, if present, your family. We cannot and do not guarantee or promise that you will receive any benefits from this study.Your decision whether to participate in this study will not affect your opportunity to receive services now or in the future from Care Net.
PAYMENTS: You will receive a thank you incentive for completion of all research requirements including three separate research surveys and attendance at the Impact Event training day. The incentive will be a gift card for use in a local grocery store where groceries and fuel can be purchased. Incentive Packages are available for this research event only and not for future Care Net Impact Events.
PARTICIPANT'S RIGHTS: If you have read this form and have decided to participate in this project, please understand your participation is voluntary and you have the right to withdraw your consent or discontinue participation at any time without penalty or loss of benefits to which you are otherwise entitled. The alternative is not to participate. You have the right to refuse to answer questions. The results of this research study may be presented at scientific or professional meetings or published in scientific journals.
