IHDA's Asset Management department requires developments to communicate updates for key contact information. This form is for updating contact information for the following:

1) Owner *
2) Management Agent *
3) Property Management
4) Occupancy Reports
5) Quarterly Reserves Statements
6) Billing **

Sources to help confirm current contact information on file with IHDA include Annual Compliance Package, on-site reviews, periodic communications with your Asset Manager or other IHDA staff, reserves statements and invoices, etc.

* For an Owner or Management Organization change, complete Special Assests Request Form To update Owner or Management Agent contact information other than a new organization, continue completing this form.

** This is where the monthly invoice would be sent, if applicable.
