Food Insecurity Survey
RequiredRequired Question(s)
1.
The recent COVID-19 pandemic has highlighted some key areas of need in the communities we serve--the largest being food insecurity. Since the 1889 Foundation and the United Way began working together on emergency funding requests in March, the largest amount of dollars distributed have gone directly to food pantries, community meal programs, and food delivery services for our most vulnerable residents.

We recognize that there are community groups who are also working to address this issue through their generosity and volunteerism. To get a better idea of the need, we are asking that community groups or individuals that provide food services (pantries, meals, backpack programs, etc.) fill out this short survey, which will help us understand how we may be able to work together. Your answers will be kept confidential.

To get started, please enter your name or the name of your organization below.
