Levitzacks' Employee and Visitor Health Screening Protocol and Questionnaire

Please be advised there is a new Health Screening Protocol regarding COVID-19 screening of employees and visitors prior to allowing entry into our office.  This questionnaire must be completed daily or as needed prior to entering our office suite for the first time each day.  If you have not completed the questionnaire, you will not be able to come into the office.  (For LevitZacks' employees, once we implement temperature checks, this questionnaire will only be needed before 8:30 am and after 11:30 am M-F and any time on the weekends.)

-Congestion or runny nose, if symptoms are believed to be allergy-related, you may come to   work

-Nausea or vomiting 

-Diarrhea

-Exposure within the last 14 days to individual(s) who have recently tested positive for   COVID-19

Starting June 15, 2020, all employees and visitors who are physically going into the office must complete this screening on a daily basis, prior to entering our suite until temperature screenings are implemented.
