Planning Survey-WISH Community School Families-Spring 2020 Session-Revision 3
Dear WISH Community School Families (with a Child in TK/K and/or 1st-5th Grade):

As part of our Continuous Quality Improvement Program and Program Planning Process, we periodically request Families to complete a Survey so that we can get feedback to improve our Programs.  We are planning to teach our Enrichment Classes at WISH Community School during the Spring 2020 Session (March 4-May 29, 2020) and the Fall 2020 Session (September to December 2020), with Days off for School Breaks.
PLEASE COMPLETE THIS SURVEY at your earliest convenience; by Saturday February 15, 2020 if possible.  It should take about 10-15 minutes to complete.  Your feedback will help us to design and offer programs that you may be interested in for your Child for these next sessions of Enrichment Classes and Camps.  If completed by February 15, you will be entered into a FREE Drawing where you can win a $100 Gift Certificate that can be applied to our Enrichment Classes at WISH and/or our Programs (Camps and Classes) at the Dockweiler Youth Center.
If you have any questions about this, please Email CoachStu@att.net and/or call 818-597-1550.  Thank you.
