Care Design NY recognizes that, due to COVID-19, everyone is navigating new norms and expectations. Many families, whose children have disabilities, are experiencing particularly unique and challenging circumstances related to their child's special education needs. Families may ask questions or share concerns with their Care Manager who can assist them to find resolutions, seek additional guidance for families, and/or connect families with additional community resources. Care Design NY also provides regular opportunities for families to attend online learning sessions and open forum discussions focused on special education related topics.

To help guide our work and commitment to provide meaningful support and resources, we ask you to take 2-3 minutes and respond to the following questions. This survey is designed for Care Design NY members (through age 21) and their families living in the following regions/counties:

~ Capital Region 1, 2 (Albany, Columbia, Montgomery, Rensselaer, Saratoga, Schenectady, 
   and Schoharie);
~ Lower Hudson Valley (Rockland and Westchester);
~ North Country (Clinton, Essex, Franklin, Fulton, Hamilton, St. Lawrence, Warren, 
   and Washington);
~ Upper Hudson Valley (Dutchess, Greene, Orange, Putnam, Sullivan, and Ulster).
